ICU Nurse

Work is great, I really love working in ICU. I have more good days than not. I have to use my nursing skills/judgement with patients. My patients are touch-and-go rather than waiting for a nursing home to accept them. Many days it's so intense that I want to rethink my choices, but I'd still choose it.

The intensivists (ICU doctors) are an odd bunch. There's three of them. Doctorita is a no frills, no extra movements. She comes in like a whirlwind and turns off sedation and leaves silently. Then, I'm usually wondering why Pat is wide eyed, flailing and arching off the bed. Thirty minutes later she has 95% of the people extubated and half-way out of the ICU. Don't get me wrong, she's a great doctor and doesn't give people cushions when they need a good slap. I've been working there for like 6 months, and I'm still surprised when she comes around. I've gotten a little better at expecting her movements though. I hold off on any sedation until mid-morning (otherwise "they're too sedated, don't give them that much") and try to get ahead by planning on them being extubated and out of bed.

Doctorio doesn't care about anything, but cares about everything. His most used phrase is "I don't even know why we're doing this!" "Why this?" and "why that?" but then, "I don't care, what do you want me to do??" He can be super nice and so easy going, but then WHAM! something spits in his coffee and he's McCrankypants. But, to be fair, we've had some nice chats during some procedures. He laughed at my kid's name, I laugh and chortle at his. And he's very down-to-earth with family and patients. He gives slightly more cushion that Doctorita.

Then we have Señor Doctor. I think his goal is to do as little as possible, unless he can get other people to do it for him. He likes to keep people intubated and sedated if possible. Now, he makes for a pretty easy day, but not always in their best interest. Everyone knows that when he's on for the week that everyone stays intubated and sedated. He loves to give out water flushes though, haha.

But I love it. I have some irking things when I work with certain people, but who doesn't? It's not as bad as I was thinking, working with some nurses. I do know how to work my days when I work with the select few. *cough*

But like I said, I would still make the same choice to go.

Voices in my head

I started my car this morning in the dark. It was before 0630, and the sun wasn't up. I started the car and heard voices. I turned up my radio to hear what they were saying, but realized it was set to "AUX" meaning my iPhone. It was in my pocket. Not plugged in. I turned it off, just to make sure. Nope, the voices were still talking. I panicked a bit, my HR spiked and my imagination went into hyperdrive.

Then I realized that the kids' DVD was turned on and it was Flynn Ryder and Rapunzel having a conversation. Then I was ok. But I really panicked before I knew. 

The birds

There's this flock of birds that are always circling the hospital and hanging out on the roof. I wonder what they're waiting for?

I can't help but think of the Alfred Hitchcock film about the birds that attack all the people. I have to stop myself from running inside. I think I may be scarred. 

Dickies summer Scrub line from Uniformed Scrubs

Back a while ago, kinda a long while ago...my bad...Uniformed Scrubs contacted me to review one of Dickies' medical scrubs for their spring and summer collection.

Since our hospital has gone to solid uniform colors, pickings for nurse scrub tops are slim. Well, they really aren't, but because any color/contrast that's not white or navy is not allowed, it seems like it. When Uniformed Scrubs contacted me to review a top of Dickie's spring line, I was happy to potentially find a favorite line, that would have variation with the style. Unfortunately, I wasn't able to get the color that Local Hospital wears, so I couldn't wear it at work. I did wear it around the house with the boys, and believe me, I did a LOT of movement in it. 

It's so so soft, almost as soft as a uber-expensive pair of scrubs I have. It's also long enough to not show my baby fluff. No drafting allowed here! It's made of thicker fabric than most, but that's good. I have this one top that is super thin, so I have to wear another shirt underneath. This one, would be fine without any extra clothes. And the pockets! I usually have my phone with me, because of my kids being at daycare. I hate to wear this one pair of pants because it doesn't have hip pockets, but this top would be my solution! It had double pockets on each side, so I can wear those pants with this top and not have to worry about my phone ending up like it got into a fight with my scissors. There's also a sleeve pocket, I've seen some people use their sleeves for hanging their badges on, not my cup of tea, but it'd work we'll. maybe a pen or two? I've also seen some wear our Nurse Tracker on it. Keeps it out of the way, but easily scanned by the system.

It's got cute details also- ribbon lining the slits at the waist, contrasting stitching here and there. It's pretty affordable too, as scrubs go. It comes in the usual colors, so if your hospital is like mine and gone to uniform colors it will work for you :)

If I had to start again with my medical scrubs, I'd totally buy one brand, especially to make sure that my shades of navy were all the same shade. And buying from one site, like Uniformed Scrubs, ensures that you can save on shipping, and exchanges would be so much easier with one site.

Most Important News

At work yesterday, the 6 pm news came on. I forget what channel, but I was in Long-Term Trach Guy's room doing something. Anyway, the 6 pm news came on, and the first two stories were studies that had been done and needed to be reported. The first one was that binge-drinking may lead to a increased risk if alcohol dependence. Seriously?? Did you know that? I had no clue!! The seldom story was almost as mind blowing as the first. Ready? People may feel more inclined to loose weight if there's a payout involved. ERMAGERD!! (Haha, I've always been wanting to use that!) There you go. Prime-time news stories. Tell me a few things. First, who got paid to research that? Second, who decided "wow, this is super important stuff, let's put it at 6 pm so everyone will notice it!" And third, did the reporter laugh and snort when she saw the "headlines" she had to report and be excited about? That would have been hard to do.
"Ok, let new what stories I have tonight. I will be excited about them! Hmm, ok. What's this? Binge-drinking may increase the risk for alcohol depen-what?? I have to say this? Can't I report something exciting? What about this 5-block-sized asteroid? Can I tell them about this? I'll make them all pee their pants!"
"Nope, that's Sven's story, he's the space guy. You're the studies guy. You get this one."
"Ffftt. Fine. What else do I have? Weight-loss, that's interesting for everyone. Ok, people may be more likely to loose weight if there's a monetary incentive. Umm. Sure, I'd even gain weight if you'd pay me! This is that news that supposed to be on at 2 am when no one is watching!"
"Oh no, that's where we put Sven's story about the asteroid, because it's not going to hit earth. THAT'S our 2 am story. Your stories are the earth shattering studies."

Haha. My coworkers and I had a great laugh. Nevermind I didn't get to leave until almost 8 pm.

Bait and switch

The past two days I've worked, I've asked for a bowl of oatmeal and brown sugar for breakfast. Well, they must be outoif oatmeal, because I keep getting something else. The first day, the lady was like, "we're out of oatmeal, here have a orange or a banana." Seriously?! Ok. Whatever. At least it's something. Then, the next day. I was hoping for oatmeal, please? Nope. But the guy (a different person was the key) brought me a plate of French toast! You guys! I love French toast! I like the oatmeal here too, but that's ok, I'll take French toast :)

No patience for patients

Still alive, yo.

Patient of the day: "Are you coming back in the morning? Because I'd like someone who already knows me to be my nurse" was said to me about 5 minutes after I met her. I met her at 7:15 pm, as my shift was ending. I guess she thought we had a connection going on.

Runner-up patient of the day: bed-ridden, 350 pounds, massively swollen legs, necrotic ulcers on her abdominal panis, multiple pressure sores on her back, butt, and posterior thighs, has a long-term trach, incontinent of poop, and won't do anything for herself. She thinks she gets to go home because she doesn't want to go to rehab/nursing home. Who will care for her? Her parents who are in their 70s. Her momma can barely walk without falling, her daddy is in a neck brace. It takes three strapping nurses to turn her over, amid her many shouts of "I don't want to turn, I'll be ok sitting in my sh*t, leave me alone. Yeah, of course she'll be fine at home.

Confession time!

So, with all the Mommy wars going around, I just want to put it out there why I'm a good mother to my kids. They're in no particular order, just as I think of them. And this not an all-inclusive list!

I use medications, like Tylenol and Benadryl for both of them.

They get their vaccines.

They use cloth diapers.

They use disposable diapers.

I work +12hrs a day, 3-4 days a week.

Some times, on my days off, I wish I was at work.

I carry (carried, too) my kids in my arms, in a baby carrier, and in their infant carseat. And they ride in the shopping carts without a protective cover!

I love coming home and seeing my son run to me yelling "Momma!!"

I love seeing my husband come home so I can pass of my kids to him for a break!

I hardly ever cook. Pancakes, French toast, and grilled cheese are weekly meals.

We don't eat organic.

I have laundry piled up all over my bedroom. Ha, my living room too.

I bathe the kids when they're cranky, not caring if they're dirty or not.

I had an epidural with both boys' labors, and breastfed them until a year or so. (still nursing the little one)

I used formula with both of them. And pumped while working full time (12+hr shifts!)

They're both circumcised.

My little one cries a lot. Nothing I do changes or helps it. So he cries.

My big one watches too much TV, but he can count to 13 and knows all the letters in the alphabet and numbers 0-10, so I'm ok with it. Thank you Sesame Street!

I usually have dirty dishes in the sink and stinky sippy cups under the couch.

We eat in the living room. Often.

My 3yo isn't potty trained yet. And my 11 mo doesn't sleep thought the night yet.

My 3 yo has a Bible verse memorized. It's "God loved us and sent his Son." and I'm super proud of him.

We don't spank often, but when we do, it's well-heeded.

I could go on. I'm not the perfect mom, or wife, but I'm perfect for my family. If someone tries to attack my choices and tell me I'm a bad parent because of what I do, I'm going to leave that person as fast as I can. I berate myself enough as it is, I don't want or need any one else doing it for me.

At the end of the day (or the middle of the night) they know I love them and when they call, my arms are around them tight!

Life as I know it

Life with a toddler/preschooler is never boring. I am woken up by a nasally breathing. When I open my eyes, I see his head peaking over the bed and gazing at me. "Mama?" he whispers. I scot into the middles of the bed and and bombarded by his growing body. Some times he'll lay there for 2 minutes. Sometimes he's asleep. But usually he curls up next to me. I try to remember when he was a nursling.
And then the day starts. At 630, or 6. Or 645. But usually 630. He asks for milk. But it come out "mutch? Wam mutch?" He carries his cup to the couch, curls up under a blanket and drinks his milk. He's my baby again for the 2 minutes it takes him to drink the milk. Then the craziness begins. GEM wakes up, BAM wants breakfast, GEM wants breakfast, I need breakfast. BAM usually asks to watch a movie. Recently it's been "rat" or "scare" or "balloons" which translate into Ratatouille, Monsters Inc (or it could be Veggie Tales), and Up. Gavin is happy to watch either of those, so long as he can see and feel me!
Nap time for Gavin means shower time for momma :D Brendon entertains himself while I relax. I'm always apprehensive when I get out because I'm not sure what he'll have gotten into. Usually everything is fine, but there's the time he dipped (rather, jumped??) into the peanut butter. Of when he climbed into Gavin's crib, effectively destroying the morning nap. Or what about the time he dusted the kitchen with cinnamon? It smelled lovely but was such a mess.
But, in spite of all the challenges my boys are apart and together, I wouldn't have it any other way.

Should be called "people of Water Country USA"

We went to Water Country USA Saturday evening, and I really had to wonder at the peoples attire. Now, I know it's a water park, and many people are comfortable enough (some times TOO comfortable) with how their body looks to wear what they wear. But these made me ponder. I'll keep the mental images to a minimum :)

• a tank top UNDER a bathing suit, so it wasn't too low cut. You want to know how I know it was a tank top and not an added triangle of fabric? because it stuck out underneath the bathing suit. I'm very familiar with not wanting to show all of one's cleavage, but if you're going to wear a bathing suit and NOT like what it shows, wear the tank ON TOP!
• a "one piece" held together by a thin strip of material up the front. You those cars that are "convertible," but have a T piece in the middle? Yeah, like that. Just go for the bikini because it's all out there anyways.
• jeans and a polo. He did have his swim trunks tucked under his arm, and in a strange coincidence I saw him later wearing his swim trunks, but why come in jeans and a polo? in 100+ degree weather?!
• a strapless top (tankini type) with some sort of swimming pants. No joke. they went almost to her ankles (with cuffs) and she was in the water up to her knees, but her top was a regular bathing suit top.
• a (I'm guessing Mulsim) girl in a hijab, with long sleeves and long pants. Her top was loose enough to be in par with their dress, but her pants...think spandex yoga pants. They filled every crease...I mean, doesn't the one negate the other?

What did me and my family wear? I wore this top (which was very popular there) and some sort of running-type shorts. My son's wore rash guard shirts (because they don't need to tan yet) and swim trunks. Mark wore and athletic-type of shirt (Under Armour, I think) and swim trunks. Could we be made fun of, sure. But this is my blog for my brain's workings.

2nd Annual Flats and Handwashing Challenge: Day 7

So I'm finally posting about the end. I could do this full time if I had to. But I don't. It was hard to find the time to wash them. It was hard to find a place outside for then to dry.
If I was without electricity, I doubt I'd have access to as much water as I used. I had to soak, rinse, hot wash, hot rinse. Another two rinses after that. The last rinse water was saved for the next loads soak. I used all hit water because I had it, but I'm sure cold water would have worked, but needed more rinses to get the soap out.
I don't have any more pics, except this one at the end. I couldn't to this and work like I do. Too much time between washings would elapse and I would, and did!, have too many to wash at once.
I whole-y support buying the flour sack towels from Wal-Mart or Target vs the diaper flats. $1 each or 12 for $20. Either way, they're both cheap.
So, I did it. Did you? Could you?

2nd Annual Flats and Handwashing Challenge: Day 6

So I missed days 4 and 5 because I was working. And now today I caught up on all my washing. What an appropriate topic, my wash routine!
I drop a wet diaper in a bucket of water. I keep the bucket in the tub. When I have several diapers I use a designated spoon to agitate the diapers, but I think using my hand works better. Then I wring and refill the bucket with hot water. One more rinse, then a table spoon of Rockin Green for a load, which ends up being 4-6 diapers. I tried using 1/2 tbspn, but they were still stinky. Then I do two agitating rinses with hot water and wrong between each one.
Between wringing diapers and taking the stairs at work (the elevators were on the fritz) I'm going to be so muscular!!

I rolled each diaper and cover in a twirl and had my "assistant" help stomp them! But that was kinda dumb. Now I have a pile of wet towels!

And then I hang them up! Outside if it's a morning load, inside around the house if it's a nighttime load. I'm still waiting for a clothes line, so I improvise :)

2nd Annual Flats and Handwashing Challenge: Day 3

So, Day 3. In case you've just stumbled on my blog, I'll let Kim from Dirty Diaper Laundry explain.

Today is the third full day of the Flats and Handwashing Challenge.  As a reminder, this is a 7 day Challenge where participants can only use flat cloth diapers and have to ditch their washer and dryer in favor of good ol’ fashioned handwashing.  We are close to 500 participants this year and many are blogging their journey.  You can find their posts linked at the end.

Why? Because almost every parent has had to buy diapers for their child. Some families can't afford diapers as often as needed, so they choose to either leave the baby in a diaper longer, or let bills go unpaid to buy diapers. These participants are showing everyone that, with a little bit of work, you can diaper a baby for less! If I had to do only flats, I would probably upgrade my bucket and spoon to a bigger bucket and some sort of plunger/paddle, or even a washboard.

How am I using my flats? I mostly fold them in half, then in thirds then in half again to make a pad that fits in my covers. It's quick and easy for my baby that likes to flip and scoot away if you happen to take your hands off of him! I do like to fold them origami style, and use my snappi. I watched a few other folds on YouTube and I'm going to branch out.

For night time, I pad folded one and put it in a pocket diaper. I'd rather not have a wet cloth against Gavin's skin all night long. The pocket diaper keeps him mostly feeling dry.

I'll try to update with some pictures, but if I don't blog now, while he's napping, it won't ever happen!

So, it looks like I was supposed to save what supplies I'm using for today. Opps. Here they are again.

• 20 Flat diapers (12 Diaper Rite, 8 flour sack towels (4 Walmart brand, 4 Target brand)

• 2 Applecheeks envelopes (I'll be using these as covers, don't worry!)

• 5 gallon bucket (currently in the shower holding a diaper)

• Rockin' Green laundry detergent.

But I forgot to mention my wipes drops. I think what I have are Bummis Bum drops. Not sure because I only have a ziploc with them in it. I'm going to repurpose something to swish my diapers with, because I'm not sticking my hand in there and I don't have a new plunger. I'm sure I can find something.

I meant to post this last night, Tuesday, but as I was making my rounds on the kids, we had a poo accident that involved sheets and walls and clothes and hands and feet. So instead of blogging about day 2, I got to clean up my sleeping toddler. He would have slept through it, except that I woke him up to plop him in the bathroom.

What a way to break the hiatus, huh? Today marks the day that I use flat diapers for a week! I'm kind of thinking I'll be struggling by the time Sunday rolls back around, but let's see.

I've got to talk to the hubs to get him onboard for when he changes the baby's diaper before I get home from work.

I'm just including Gavin, because Brendon only wears diapers at night, and usually it's Pull-Ups (have I mentioned that I despise Pull -Ups? but that's a discussion for another day!)

Here's (most of) my supplies. I have another 4 flat diapers still in the laundry pile, a few lost snappies, and another Thirsties cover. And not pictured are the accessories, a bucket, drying rack (a saw-horse), detergent--stuff like that.

Today's topic of choice is why I'm choosing to do this challenge. I switched to cloth diapers to save money. While it can be debated that cloth diapers actually save money vs theoretically (because the diapers are so cute, addicting and scream BUY ME!!), I want to prove that you can cloth diaper on a low budget and save money! I'm going to list the prices of the covers and flats and accessories. Although most of what I have has been bough used, I'm going to list the retail value.

And, as a work-out side the home mom, I'd like to show you it can be done!!

Flats: 12 for $17.50 (Diaper Rite), 8 for $8 (Walmart and Target)

Fleece covers: $6-9 each (the camo cover I made, and the blue one was bought used, but I think you can find a new one for about $8

GroVia covers: $16.95 each (the seaside I bought new from GroVia, the owl I bought from BabySteals, and the nature I bought used)

AppleCheeks covers: $20.50 for the print (pictured) $19.00 for the solid. Not pictured.

Retail Value is $175.35. While that is quite a bit of money for a "simple" diapering system, I actually only spent about $100 on all that. I fully advocate for used cloth diapers. Yes, it's a little disturbing at first (it was to me!), but I brought them home and washed the snot out of them! lots of hot water and sun!

NO! Not the finger!!

ok, so my finger . . . I was working, last week Tuesday. It was just about 2. One of my coworkers was headed to lunch when an ultrasound tech needed help positioning a patient to get a better view of what she was looking at. The girl was going to do it, but I told her to go eat and I'll help. Silly me. I grabbed that turn sheet and pulled like I usually do. *pop* went my finger and *tear* went my eyes. Now, I was just thinking that it just popped a knuckle or a joint. But I had to take a minute and work it off. I tried to play it dow, for the tech (who wants to admit they just hurt themselves in front of anyone!?), but I couldn't grab that sheet easily. But I did it, and I pulled him over, like the nurse I was. After that, I just went about my business, but realized that I couldn't bend it easily or even grab anything. I tried pushing a syringe, to inject some meds, but I couldn't do that either.

Then I lost it! I started crying like a sleep-deprived baby. I mean, it didn't hurt that bad, though it did ache and throb. I put some ice on it, and everyone (as in every singe person in every single room I went it) asked about my finger. It's nice to be thought of :)

Long story short, the urgent care clinic doc confirmed the break with an x-ray, and I got an appointment to go see the hand orthopedic doc, who put it in a split, said I'd be on modified work for 8-10 weeks (eight to ten WEEKS!) and sent me off to get some occupational therapy for it. A week and a half later, the bruising is old bruising is gone (new bruising is here though!), the swelling is gone (except at the end of the day), and it's stiff as can be (until I've done a few exercises).

I am "lucky" enough to get to go to occupational therapy twice a week. Think fine motor skills, like picking up pegs and putting them in holes. I have a custom made splint that is frustrating me at every single thing I do. It makes my finger 2-3 times it's normal thickness, hits everything I walk by, attracts water to get soggy, and sticks to the sticky side of velcro (like diapers and coats!)

For the next 6-10 weeks I get to do paperwork and GO-fering. I'm hoping at the 6 week follow-up visit I'll get to go back to normal work, but I kind of doubt it. I've done admission documenting audits (we stink at documenting the education!), blood transfusion audits, the daily assignments, the weekly assignments, the month-and-a-half-ly assignments, I've passed out tootsie pops and mirrors (to remind you to Protect Our Patients Skin- POPS). I've checked the code cards, the hemoccult/gastroccult boxes, the glucometer strips and controls, the fridge temps, and the OmniCell discrepancies. I've laminated and cut out the laminated thing. I've made hundreds of photocopies. I've walked up and down the steps at good 20 times EACH DAY (but don't forget -- for every elevator ride, I take the steps!) I've worked more on "light duty" then I usually do on the floor.

Well, that's not right. It's more busy work. I do more grouped work on the floor. Now, I'm running between the four pods all day long. I can't seem to find a good place to put my pumping bag. Mommy's Place is on the second floor-I don't want to store it there because--oh, I guess I could. There's a cabinet behind the fridge that no one who needs the room would use. I'll have to try it. But any ways. I couldn't keep on the third floor, because I do most of my work on the fourth floor, but my locker is on the third floor. I can't seem to find a good flow for the day yet.

My finger is numb right now. Not fully numb, but enough that it feels weird. I can't wait until it's healed.

UV coating, with rounded corners. (huh? yeah, me too! LOL) but get this! They'll be ready to ship to you in 4 business days. FOUR!! talk about speedy Gonzales! 

Please, ignore the mis-spelling, the mis-capitalizations, and such. I'm typing with a broken finger in a splint, and it's very frustrating! (yes, more on that later too! "later")

Back to business: I wasn't paid money for this giveaway, it's all my honesty, but to thank me for bringing them business, I'm going to get a set of post cards too. I'm thinking of teaching Brendon about mailing letters to his friends and family!

Misread words

The other day, I was in line at the cafeteria. I glanced at the menu and saw "carbonara with fetticinni and" another word that I could not figure out the last word. Like I said I only glanced at the sign. I could have looked again. I could have read it twice. No one said that I only get one read per line. I was stubborn, I tried do hard to figure out the last word. I knew it had b's and c's and a few vowels in it.

I finally allowed a second glance, because the line hadn't moved. "Brocolli" duh!

Isn't it silly how one misread, misseen or even misheard word can change the meaning, or cause a commotion?
